Our Bridges to Care Program is designed for clients who are transitioning back to their homes from a hospital. Once you or your loved one is ready to return home, our care staff will visit you to evaluate what services we can provide to make the transition easier for you. Often times, a little extra help is just what is needed to get back on track.

A growing number of adults are currently taking at least one medication while many adults take more than four prescription medications at the same time. Remembering to take medications on time can become challenging, especially for the elderly. Bridgewater care providers can assist with reading labels and reminding seniors to take their appropriate medications.
By having Bridgewater Care providers help with washing, drying and putting away the laundry, seniors can often conserve their energy for more important tasks. Many times these services are done in the home and other times assistance can be provided to clients at a Laundromat.
Keeping up with housekeeping can be a challenge for many seniors. Bridgewater Care Providers can complete light housekeeping services such as dusting, sweeping, vacuuming, mopping floors, cleaning bathrooms and kitchens. Care Providers also help with organizing drawers and closets and making sure things are picked up to avoid any slips or falls.

Many seniors do not have the opportunity to eat a well-balanced meal, and some even participate in cooking activities so they become involved in as much or as little as their ability and desire allow. Many seniors have a problem getting to the store, picking up prescriptions, running errands, going to the beauty salon and getting to their doctor’s appointments. 24-Hour Home Care Services
There are times when clients need around the clock 24 hour services. Depending on the need, Bridgewater Senior Home Care can coordinate care-giving with shifts of employees who can provide specific services. In many instances, this type of continuous in-home care service is provided for clients who are at risk when unattended, such as persons with Alzheimer’s disease and other dementia-related conditions.
Restroom / Incontinence Care
Maintaining a senior’s dignity is very important. Bridgewater Senior Home Care has trained Care Providers who understand how it may be uncomfortable to need assistance with restroom use or incontinence care. That’s why there is a high degree of compassion and concern about making sure the client’s dignity is maintained when providing these services.
Bathing & Hygiene
Seniors often need help with bathing and basic hygiene. These services are important in maintaining the client’s physical well-being and often also can have an impact on their mental well-being.
